3 questions about part time job application for college student?

hi all. i’m college student who’s trying find part time job @ mall. i’ve had 2 part time jobs before. have 3 questions job applications i’m turning in right now:
(1) 1 application asks “minimum salary desired.” i’m working part time, not full time. put “n/a”?
(2) reasons leaving 2 previous jobs, i’ve read here on yahoo answers can put “looking better job opportunities.” ok put? sounds you’re saying old jobs sucked. there better way phrase this? weird if put reason both previous jobs? first job, quit b/c far commute, “commute” ok excuse leaving on application?
