Stage fright help me I've ran out of ideas ?!?

ok first got stage fright when 11 years old
remember extremely , ever since haven't been able perform in front of anyone. gets in way, im still in school. need started making important choices careers , such problem holding me things wanted do.
these symptoms
trembling legs , hands
sweaty hands
feeling sick
on rare occasions perform voice crack , it'll sound quite shaky well.
i've tried , cant rid of these feelings please don't know else ?
